How to Care for Surgery Scars: A Step-by-Step Guide

When the stitches come out, the real work begins. Here's how to support your skin through every phase of post-surgical healing.

Surgery scars heal in stages, and the care your skin needs in week one is very different from what it needs at month six. The goal of a good scar care routine isn't to erase what happened — it's to support your body as it rebuilds.

Phase 1: The first two weeks

Follow your surgeon's wound-care instructions exactly. Keep the incision clean, dry, and protected. Do not apply silicone gel, scar tape, oils, or actives directly to an open wound — wait until your provider confirms the incision is fully closed.

Phase 2: Weeks 2 to 6 — starting scar care

Once your incision is closed and your surgeon has cleared topical products, this is the window where scar care matters most. Medical-grade silicone is the most studied, dermatologist-recommended approach for flattening, softening, and fading new scars.

  • Apply silicone scar gel in a thin, even layer twice daily.
  • Use silicone scar tape overnight or under clothing for continuous support.
  • Protect the scar from direct sun — UV exposure can darken healing tissue for up to 12 months.

Phase 3: Months 2 to 12 — patience and consistency

Scars remodel for up to a year (longer for some skin types). Consistency beats intensity. Keep using silicone daily, keep sun-protecting, and check in with your body without judgment. Healing is not linear.

When to call your provider

If your scar becomes raised, itchy, painful, dark, or starts to extend beyond the original incision line, talk to your surgeon or a dermatologist. Early intervention for keloid and hypertrophic scarring makes a real difference.
